1. If you request your carte vitale online (ameli.fr), the documents will be requested as pdf, scans or photos so you will not need a hard copy.

Now, if you fill out the cerfa form by mail, you will need to send the stamped hard copy document.

By experience, the ameli services can at any time request the hard copy as well later on in the process.

I would suggest you pick up the hard copy as well, it is included in the price, so take advantage of this!

 

2. Once you receive the digital copy, you will have a link to book an appointment.

If you wish to have your documents signed and stamped as well (with number vu ne varietur),
